Blood tests cannot directly diagnose telogen effluvium. However, they can help identify medical conditions, nutritional deficiencies and hormonal problems that may trigger or prolong diffuse hair shedding.

A healthcare provider may consider testing when there is no obvious trigger, shedding continues longer than expected or symptoms suggest an underlying issue such as iron deficiency or thyroid disease.

Can a Blood Test Diagnose Telogen Effluvium?

No single blood test confirms telogen effluvium.

The diagnosis is generally based on:

  • Sudden or increased diffuse shedding
  • The timing of the shedding
  • A possible trigger several months earlier
  • Scalp and hair examination
  • The absence of signs pointing to another form of hair loss

A clinician may also use a hair-pull test or trichoscopy to assess active shedding, follicle miniaturization, broken hairs, scalp inflammation and other diagnostic clues.

Bloodwork serves a different purpose. It helps investigate why the shedding may be happening.

For example, testing may identify:

  • Iron deficiency
  • Anemia
  • Thyroid dysfunction
  • A nutritional deficiency
  • Signs of systemic illness
  • A medical condition contributing to continued shedding

Telogen effluvium can still occur when every laboratory result is normal. Illness, surgery, childbirth, psychological stress, medication changes and rapid weight loss can disrupt the hair cycle without producing an ongoing blood-test abnormality.

When Blood Tests May Be Appropriate

Laboratory testing may be more useful when:

  • There is no clear trigger for the shedding
  • Hair shedding persists for more than six months
  • You have symptoms of anemia or thyroid disease
  • You have experienced rapid or significant weight loss
  • Your diet is highly restrictive
  • You have heavy or prolonged menstrual bleeding
  • You are postpartum and have additional symptoms
  • You have a gastrointestinal condition that affects nutrient absorption
  • You take medications that may affect thyroid or nutritional status
  • The shedding is severe, recurrent or unexplained
  • Your scalp examination does not fully explain the hair loss

Testing may be less extensive when the history strongly supports an acute episode of TE, such as shedding that begins several months after a major illness or surgery and is already improving.

A dermatologist or primary care clinician should determine which tests are appropriate rather than ordering a large panel without a specific clinical reason.

Complete Blood Count

A complete blood count, commonly called a CBC, measures several components of the blood, including:

  • Hemoglobin
  • Hematocrit
  • Red blood-cell count
  • Mean corpuscular volume
  • White blood cells
  • Platelets

The CBC is commonly used to identify anemia and other blood abnormalities.

What a CBC Can Show

Low hemoglobin or hematocrit may indicate anemia. Mean corpuscular volume can also provide clues about whether anemia may be related to iron deficiency, vitamin B12 deficiency, folate deficiency or another cause.

However, a normal CBC does not prove that iron stores are adequate.

Iron deficiency develops in stages. Ferritin may decline before hemoglobin falls low enough to meet the definition of anemia. This means someone can have depleted iron stores while their CBC remains within the laboratory reference range.

Ferritin and Iron Studies

Ferritin is a protein that stores iron. The level of ferritin in the blood is commonly used to estimate the body’s iron reserves.

Low ferritin supports a diagnosis of depleted iron stores. Iron deficiency can occur with or without anemia and may contribute to diffuse hair shedding in some people.

Risk factors include:

  • Heavy menstrual bleeding
  • Pregnancy or recent childbirth
  • Gastrointestinal blood loss
  • Vegetarian or highly restrictive diets
  • Poor dietary intake
  • Malabsorption
  • Bariatric or gastrointestinal surgery
  • Frequent blood donation
  • Inflammatory bowel disease
  • Celiac disease

A 2026 systematic review and meta-analysis found that ferritin levels were lower, on average, among people with telogen effluvium than among control groups. However, the included studies had substantial variability, and an association does not prove that low ferritin is the cause of every TE case.

Ferritin Is Not the Same as Hemoglobin

These markers answer different questions:

  • Ferritin estimates stored iron.
  • Hemoglobin measures an oxygen-carrying protein inside red blood cells.
  • Serum iron measures iron circulating in the blood at the time of testing.
  • Transferrin saturation estimates how much of the iron-transport protein is carrying iron.
  • TIBC reflects the blood’s capacity to bind and transport iron.

A person may have low ferritin while hemoglobin remains normal.

Ferritin Can Be Affected by Inflammation

Ferritin is also an acute-phase reactant. It can increase during inflammation, infection, liver disease and certain chronic illnesses.

A ferritin result that appears normal or high may therefore be harder to interpret in someone with active inflammation. A clinician may review ferritin alongside transferrin saturation, CBC, inflammatory markers and the broader medical history.

Is There an Ideal Ferritin Level for Hair Growth?

There is no universally accepted ferritin target that guarantees hair growth or proves that shedding is iron-related.

Online recommendations often promote a specific ferritin number for hair regrowth, but these proposed targets are not consistently supported by clinical guidelines.

Results should be interpreted using:

  • The laboratory reference range
  • Symptoms
  • Hemoglobin and red-cell findings
  • Menstrual or bleeding history
  • Iron studies
  • Inflammation
  • The individual’s medical history

Low ferritin should be investigated and treated appropriately. However, taking iron solely to reach an internet-based “hair target” can lead to unnecessary supplementation and iron overload.

Thyroid Blood Tests

Both an underactive thyroid and an overactive thyroid can contribute to diffuse hair changes and shedding.

Thyroid testing may be appropriate when hair loss occurs alongside symptoms such as:

  • Unexplained fatigue
  • Weight gain or weight loss
  • Feeling unusually cold or hot
  • Changes in heart rate
  • Constipation or frequent bowel movements
  • Dry skin
  • Muscle weakness
  • Menstrual irregularities
  • Anxiety or tremors
  • A personal or family history of thyroid disease

TSH

Thyroid-stimulating hormone is generally the first blood test used to evaluate thyroid function.

TSH is produced by the pituitary gland and signals the thyroid to produce hormones. An abnormal TSH result can suggest hypothyroidism or hyperthyroidism, although interpretation depends on the clinical context.

Free T4

Free T4 measures the unbound portion of thyroxine available to the body.

A clinician may order free T4 when:

  • TSH is abnormal
  • Pituitary disease is suspected
  • Symptoms strongly suggest thyroid dysfunction
  • The person is already receiving thyroid treatment

Free T3 and Thyroid Antibodies

Free T3 is not routinely required in every evaluation of hair shedding. It may be helpful in selected cases, particularly when hyperthyroidism is suspected.

Thyroid antibodies may be ordered when an autoimmune thyroid condition such as Hashimoto’s thyroiditis or Graves’ disease is being considered.

The standard blood test for vitamin D status is 25-hydroxyvitamin D.

Vitamin D is involved in multiple biological processes, including immune regulation and hair-follicle activity. Research has found lower vitamin D levels in some populations with telogen effluvium and other hair-loss disorders.

The 2026 systematic review and meta-analysis also found lower average vitamin D levels in TE cases, but the studies were heterogeneous. This means low vitamin D may be relevant in some patients, but a low result does not independently diagnose the cause of shedding.

Testing may be more reasonable when someone has:

  • Limited sun exposure
  • Darker skin and other deficiency risk factors
  • Malabsorption
  • Osteoporosis or bone symptoms
  • Chronic kidney or liver disease
  • A restrictive diet
  • A previously documented deficiency
  • Other symptoms or medical reasons to assess vitamin D

Correcting a confirmed deficiency may support overall health, but taking high doses without testing or medical guidance is not recommended.

Vitamin B12 and Folate Testing

Vitamin B12 and folate are involved in DNA synthesis and healthy red blood-cell production.

Deficiency can contribute to anemia and other systemic symptoms. Whether either deficiency is directly responsible for hair shedding varies by individual and should not be assumed from hair loss alone.

Testing may be considered when someone has:

  • A vegan or highly restrictive diet
  • Pernicious anemia
  • Celiac disease

Zinc Testing

Zinc is involved in protein synthesis, immune function and tissue growth.

Severe zinc deficiency can cause hair loss, skin changes, impaired wound healing and other symptoms. However, routinely testing zinc in every person with diffuse shedding is not always necessary.

Testing may be considered when risk factors include:

  • Malabsorption
  • Chronic diarrhea
  • Bariatric surgery
  • Severe dietary restriction
  • Alcohol-use disorder
  • Long-term parenteral nutrition
  • Certain chronic illnesses
  • Symptoms consistent with zinc deficiency

Serum zinc has limitations. Results can be influenced by fasting status, inflammation, time of day and recent meals. A result should therefore be interpreted in context.

Excessive zinc supplementation can cause nausea, gastrointestinal symptoms and copper deficiency. Zinc should not be taken in high doses simply because hair shedding is present.

Comprehensive Metabolic Panel

A metabolic panel may evaluate:

  • Liver function
  • Kidney function
  • Blood glucose
  • Electrolytes
  • Blood protein levels

It may be considered when there are systemic symptoms, chronic illness, significant weight loss or medications that require monitoring.

Total Protein and Albumin

Low protein intake and malnutrition can disrupt hair growth.

Albumin and total protein may provide useful information in severe dietary restriction, malabsorption, chronic illness or unexplained weight loss. However, these markers are not direct measurements of dietary protein intake and may remain normal in less severe deficiency.

Hormone Testing

Androgen-related testing may be considered when hair loss occurs with:

  • Irregular menstrual cycles
  • Acne
  • Increased facial or body hair
  • Infertility
  • Signs of polycystic ovary syndrome
  • Rapidly progressive patterned hair loss

Possible tests may include total testosterone, free testosterone, sex hormone-binding globulin, DHEA-S and other markers selected by the clinician.

These tests are not routinely needed for classic telogen effluvium without signs of androgen excess.

Autoimmune and Inflammatory Testing

Tests such as antinuclear antibodies, ESR or CRP may be considered when hair loss occurs with:

  • Joint pain
  • Unexplained rashes
  • Recurrent fever
  • Mouth ulcers
  • Photosensitivity
  • Other symptoms suggesting autoimmune or inflammatory disease

Broad autoimmune screening without relevant symptoms can produce unclear or incidental results and is not usually an efficient first step.

Celiac Disease Testing

Celiac testing may be appropriate in someone with hair shedding and:

  • Chronic digestive symptoms
  • Iron deficiency without a clear source
  • Unexplained weight loss
  • Nutrient deficiencies
  • A family history of celiac disease
  • Other autoimmune conditions

Testing should generally be completed while the person is still consuming gluten, unless a healthcare provider gives different instructions.

How to Prepare for Hair-Loss Blood Tests

Preparation requirements vary according to the tests ordered.

Tell Your Clinician About Supplements

Provide a complete list of:

  • Vitamins
  • Hair supplements
  • Multivitamins
  • Protein powders
  • Herbal products
  • Prescription medications
  • Over-the-counter medications

Supplements may affect test results or obscure whether a deficiency was present before supplementation began.

Biotin Can Interfere With Laboratory Results

Biotin is commonly included in hair, skin and nail supplements.

High-dose biotin can interfere with some laboratory tests and produce inaccurate results. Thyroid tests are a well-known example. Depending on the assay, biotin can make thyroid hormone levels appear falsely high and TSH appear falsely low.

The FDA advises patients and healthcare professionals to be aware of biotin interference. The American Thyroid Association advises withholding biotin before thyroid testing, although the appropriate interval can vary by dose and laboratory method. Ask the ordering clinician or laboratory how long your specific supplement should be stopped.

Ask Whether Fasting Is Required

Some iron studies and metabolic tests may be influenced by recent food intake or time of day.

Do not assume that fasting is necessary for every test. Follow the instructions provided by the clinician or laboratory.

Mention Recent Illness or Inflammation

Recent infection, inflammation, surgery and chronic disease can influence markers such as ferritin.

This information may change how the results are interpreted or whether testing should be repeated later.

Do Not Start Supplements Immediately Before Testing

Beginning iron, vitamin D, B12 or zinc immediately before testing can alter results and make it harder to determine whether a deficiency was originally present.

Do not delay medically necessary treatment, but discuss testing and timing with a healthcare professional before self-supplementing.

How to Interpret Normal Blood-Test Results

Normal bloodwork does not mean the hair loss is imaginary, and it does not rule out telogen effluvium.

TE can be triggered by an event that has already passed, including:

  • A high fever
  • COVID-19 or another infection
  • Surgery
  • Childbirth
  • Severe psychological stress
  • Rapid weight loss
  • A medication change
  • Stopping a medication
  • A major calorie or protein deficit

By the time shedding becomes noticeable, the original trigger may have resolved and bloodwork may be normal.

Normal laboratory results also do not rule out:

  • Androgenetic alopecia
  • Alopecia areata
  • Traction alopecia
  • Hair breakage
  • Scarring alopecia

When bloodwork is normal but shedding persists, the next step is usually not an unlimited number of additional blood tests. The clinician should reconsider the diagnosis, review the timeline and examine the scalp and hairs carefully.

What Happens When a Test Is Abnormal?

An abnormal result should lead to two questions:

  1. Could this abnormality contribute to the hair shedding?
  2. Why is the abnormality present?

For example, treating low iron without investigating the reason for iron loss may miss:

  • Heavy menstrual bleeding
  • Gastrointestinal bleeding
  • Poor absorption
  • Dietary restriction
  • Celiac disease
  • Another medical condition

Similarly, correcting low vitamin D or B12 does not prove that the deficiency was the only cause of hair loss.

Treatment should focus on the medical problem, not merely the laboratory number.

Hair recovery also takes time. Even after the trigger is corrected, follicles must complete the resting phase and return to active growth. Shedding may not stop immediately, and visible density can take several months to improve.
